Carriers commonly offer Disease Management & Chronic Condition Management via opt-in, a model that requires members to call for assistance. The Unified Care Connect Program uses an opt-out model. This assures every member is considered and welcomed into the program. Members may opt-out simply by emailing or calling, but opt-out rates are consistently less than 1%.
